Older-adult support.

Your home. Your rhythm. Your say.

A warm meal, a lighter household routine, someone to share the afternoon with. Practical, non-medical help shaped around you—not assumptions about your age.

Our support is practical and non-medical. Home & Heart does not replace emergency, medical, nursing, personal care, therapy, or crisis services.

Our visits do not include clinical geriatric care, medication management, bathing, toileting, lifting or transfers, or specialized supervision. If those are needed, a qualified healthcare or personal-care provider should be involved.

Getting started

Start with a conversation.

  1. 01

    Tell us what would help.

Choose a service or select “I’m not sure what I need.” Share your Ontario community and the everyday tasks you’d like help with—no medical records needed.

  2. 02

    Make a plan together.

We discuss preferences, suitability, accommodations, availability and pricing before a visit is confirmed. With the older adult’s permission, family or a trusted person can join the conversation.

  3. 03

    Welcome support at your pace.

Once arrangements are confirmed, visits follow the agreed practical support plan. Tell us what feels right and what you’d like to change; your choices remain central.

For families & trusted people

You can reach out. Their voice stays at the centre.

If you’re helping a parent, partner, friend or neighbour explore support, you’re welcome to start the conversation. Use your own contact details and briefly explain your relationship and the practical help you’re looking for.

We involve the person receiving support in decisions and seek their permission before including others or sharing their information. Family involvement is an invitation, not an assumption.

Names, pronouns, language, food preferences, household routines, and accessibility or sensory needs all matter. You can share these optionally in the booking form; we’ll discuss what we can accommodate before confirming.

Please keep descriptions general and leave out private medical, financial or other sensitive information.

The request form currently provides a preview-only confirmation. It does not send your details or arrange a call, and no visit is booked or charged.
